Event Registration
Registration begins Friday, Feb 8th, 7:00 am with breakfast.
Please choose one of the following methods to register; full payment must be received with your registration:

- Fax/Mail: Click here to download the registration form, and then print and complete. Fax the completed form to (630) 510-7176, or mail to the address provided on the form.
Registration Confirmation
If you have an e-mail address on file, you will receive an electronic confirmation. If not, a written confirmation will be mailed upon complete processing of your registration. If you have not received a confirmation within three weeks of registration, please call the College of Diplomates at (630) 510-7136 or send an email to [email protected].
Badges and registration materials will be held for you and may be picked up at the event.
Cancellation Policy
Registration packages will be refunded, less a $100 processing fee, if the COD receives written notification of cancellation by January 1st, 2013. The Executive Secretary will review refund requests made in writing after this date as a result of emergencies or other serious events. Fax cancellations requests to (630) 510-7176.
